Reveal - Case Assessment Service [SM1HDBR-REGDDEN]

This Reveal Clear Aligners offering includes HSO' Clinical Case Assessment Service to check and see what type of treatment package the patient may qualify for before a full treatment setup is developed. This is a quick assessment and is subject to change once the full treatment plan is presented for doctor review. The doctor may then make a decision if they will continue to treat the patient with aligners or refer out to an orthodontic specialist. 

To qualify for our FLEX package the patient should be pre-assessed for the following:

• NO Impacted teeth
• NO Missing Teeth
• NO Anterior bridge restorations
• NO Implants
• NO Periodontal conditions
• NO Deep bite over 50%
• NO Over Jet over 5 MM
• ONLY Mild to Moderate crowding not exceeding 5mm
• NO Anterior open Bite Class III Molar

The following records are required:   Three extraoral, front smile, front no smile, profile and five intraoral lateral left, lateral right, occlusal lower, occlusal upper.  X-rays must be a pano or FMX. For treatment to proceed digital or PVS impressions must be provided. 

Reveal Aligners are clear plastic trays that are based on the mold (using an intraoral scan or impression) of the patients teeth. 

They are a series of clear, lightweight, plastic appliances indicated for the correction of dental malocclusions in patients with permanent dentition (i.e. all second molars).
They are intended to position teeth by way of continuous gentle forces. Not to be used if the patient is allergic to plastics.

• Each aligners should be worn for 2 weeks (14 - 15 days). 
• Each aligner has to be worn throught the night and day, except during meals and while brushing.
